Difficulty can be decided solely based on Hitora, based on my demo experience.

easy- Yamato took over Germania
normal- Hitora does cute tantrum now and then
hard- Hitora orders you. You can't deny her 'strategical' orders.
insane- Hitora even orders the way your troops should act now and then. She sometimes order a ghost army to do a vital role, a.k.a. you cant achieve that goal now.
suicidal- Zhukov shows no mercy.
Currently- No good college game design student, too much anime in life

Gorchnik phases in randomly and eradicated entire countries out of existence. Then the new reign of terror begins!

I'd definitely add at the start-up screen that the game is a fictional representation rather than a accurate one lest angry fans & history buffs start flooding in after the game's release. For the map itself, I'd probably just rename each region to something completely new thus enforcing that it's only a game/fictional story. It'll also grant you greater liberty how to divide each region.
